Overview

West Pines Behavioral Hospital is a new 144-bed inpatient behavioral health facility serving Denver area residents located at 11455 Huron Street, Westminster, CO.  A joint venture between Intermountain Health and Acadia Healthcare.  Just opened in December 2024, the hospital provides comprehensive inpatient and intensive outpatient services to address the growing need for accessible, high-quality behavioral health care in the Denver metro area.

Pay range:  $25.00- $28.00

Responsibilities

​ESSENTIAL FUNCTIONS: 

  • ​Develop continuing after care plan, based on patient's needs, to promote long term health and relapse prevention.   
  • ​Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.  
  • ​Make arrangements for community resources to facilitate discharge.  Provide all needed documentation to other providers to facilitate the continuum of care. 
  • ​Act as a liaison between referral sources, patients and counselors. 
  • ​Ensure all patient discharge planning documentation is clear, concise and timely.  

​OTHER FUNCTIONS:  

  • ​Perform other functions and tasks as assigned. 

Qualifications

​EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: 

  • ​Associates degree in nursing or human services field required.  Bachelor's degree in social work or human services field preferred. 
  • ​One or more years’ experience in a clinical setting preferred. 

​One or more years’ experience in discharge planning preferred.

Preferred Qualifications:

  • Previous experience in discharge planning, care coordination, case management, behavioral health, social services, or a related healthcare setting preferred.
  • Knowledge of community resources, behavioral health services, and levels of care preferred.
  • Experience coordinating referrals, aftercare appointments, and community-based services preferred.
  • Familiarity with psychiatric, substance use, or inpatient healthcare settings preferred.
  • Strong organizational, communication, and interpersonal skills
  • Ability to establish effective working relationships with patients, families, providers, and community partners.

​LICENSES/DESIGNATIONS/CERTIFICATIONS:  

  • ​If applicable, current licensure as a LPN, RN within the state where the facility provides services. 
  • ​CPR and de-escalation and restraint certification required (training available upon hire and offered by facility.   
  • ​First aid may be required based on state or facility requirements.
